Whilst it may seem daunting to talk about discharge when the person has only just been admitted, it is important for the person, their supporters and the therapy team to have a shared goal with the commissioners about the plan for admission to our service, expected timescales and the projected outcome.

At the first review meeting, the team will be able to discuss the initial assessment, planned rehabilitation programmes and future planning even if this plan is reviewed in light of the person’s progress.

At an early designated stage in admission, a prediction should be made of the expected outcome of the programme and time scale, even though this may subsequently be reviewed.
