The OakLeaf Group offers a range of specialist services designed to meet the needs of men with a brain injury. The services are supported by our highly trained and experienced interdisciplinary staff team providing a flexible and seamless rehabilitation pathway. All rooms are single and have en-suite facilities.

Hilltop House & Hartwell Flats

A service for residents who need assessment and active rehabilitation. The residents benefit from personalised rehabilitation programmes which support their progress towards short and long term goals. Sessions are meaningful, reflect individual rehabilitation needs and incorporate each person’s unique interests. Our co-ordinated interdisciplinary team is based on site and supports a ‘sessional day’ in the dedicated therapy rooms as well as in the community. Hartwell Flats – a number of self-contained flats are available on the Hartwell site for men who can manage a greater level of independence but still require a degree of supervision and support.

Cunningham House

Is a service for residents who are admitted for an initial 12 week assessment period and for continuing rehabilitation and review. Members from each discipline work with the person to understand their levels of functioning, behaviour and rehabilitation potential. The team formulate personalised rehabilitation programmes during this period. We welcome the referrer and the resident’s family/supporters to the first review which can be scheduled at a time to suit the funder. Members of the multidisciplinary team attend the review to present and discuss the comprehensive assessment report and recommendations.

The Lodge

Is a small service for ongoing active rehabilitation formulated as a result of the initial assessment. Residents often present with complex challenges (including those linked with behaviour, risk, cognitive impairment or physical disability associated with their brain injury). All the services at the Hartwell site can admit people on Section 17 Leave with a view to a Community Treatment Order (CTO) or discharge from their section after the 12 week assessment. The team are often complimented on their ability to effectively respond to difficult or challenging situations created by changing levels of insight or presentation.

Hunsbury House

Provides a community home for up to 5 residents. Whilst the service is supported by trained staff 24 hours a day and therapy sessions during the week, the residents who live here enjoy a greater level of easy access to community activities that meets their needs and wishes. The house is in a residential area and benefits from links to public transport and nearby leisure, education and vocational opportunities. The supervision and input by the wider multidisciplinary team members enables this group of men to live in the least restrictive environment possible with a comfortable lifestyle.

Trained staff are available 24 hours a day to provide support and guidance in line with each person’s assessed needs. Residents enjoy frequent access to a wide range of activities in their local community and make key life choices whilst still benefitting from the support and supervision of the experienced, multidisciplinary staff team. These homes provide an environment where quality of life can be balanced with needs for supervision and support.

The Cotswolds

The Cotswolds provides two discrete services;  nursing and personal care for residents who have reached their active rehabilitation potential and now require a longer-term, specialist placement. Programmes are designed to maintain functional and behavioural progress, whilst focussing on improving quality of life. It also provides nursing and personal care for men with long-term, complex physical needs as a result of their brain injury. Rooms are fitted with tracking-hoists and en-suite wet rooms making life as comfortable and safe as possible. Large lounges and accessible garden areas provide a functional environment with a homely feel. Staff are trained to provide sensitive and skilled End-of-Life and Respite Care if required.
