Intensive 12 Week assessment and Active Rehabilitation Phase
Includes
Accommodation and care costs with access to the full MDT including:
- Specialised Registered Nursing
- Neuropsychiatry
- Social Work
- Neuropsychology (core therapy)
- Occupational Therapy (core therapy)
- Speech & Language Therapy (core therapy)
- Physiotherapy (core therapy)
- Other healthcare professionals e.g. GP, Dietitian
- Full structured activity timetable and intensive, co-ordinated interdisciplinary assessment.
- Risk Assessment and Management
- Comprehensive assessment review report
Excludes
Continuous 1:1 staff supervision over and above that which is naturally provided as part of the core service – any 1:1 staffing by a Support Worker will be charged at a standard hourly rate.
