Which measures we’ve used?

Risk

24 areas commonly impacted by brain injury are examined for likelihood and severity to assist with care planning.

The Oakleaf Risk Assessment developed by our expert interdisciplinary team.

Behaviour

The clinical teams gather information about positive behaviours and behaviours that could be considered challenging. By sharing this information, we understand the contributing factors and triggers to behaviour as well as what helps to positively support the resident in engagement and reducing risks.

Includes standardised measures such as:
• The Overt Aggression Scale – Modified for Neurorehabilitation (OASMNR)
• St Andrew’s Sexual Behaviour Assessment (SASBA)
• St Andrew’s Swansea Neurobehavioural Outcome Scale (SASNOS)

Functional Independence
