Assessment and specialised rehabilitation focus on providing active rehabilitation.  Unique care programmes are developed by the specialist inter-disciplinary team and centre on the needs and goals of each resident, considering short and long term goals.  Residents may also have associated complex cognitive impairments, challenging behaviours and/or physical disabilities.

The Oakleaf Group are committed to ensuring the framework of fees is transparent yet flexible to offer reducing costs at people’s needs change in line with their progress throughout their rehabilitation placement.

 

Band A1

Intensive 12 Week assessment and Active Rehabilitation Phase

Includes

Accommodation and care costs with access to the full MDT including:

  • Specialised Registered Nursing
  • Neuropsychiatry
  • Social Work
  • Neuropsychology (core therapy)
  • Occupational Therapy (core therapy)
  • Speech & Language Therapy (core therapy)
  • Physiotherapy (core therapy)
  • Other healthcare professionals e.g. GP, Dietitian
  • Full structured activity timetable and intensive, co-ordinated interdisciplinary assessment.
  • Risk Assessment and Management
  • Comprehensive assessment review report

Excludes

Continuous 1:1 staff supervision over and above that which is naturally provided as part of the core service – any 1:1 staffing by a Support Worker will be charged at a standard hourly rate.

Band A2

Continuing Rehabilitation and Review (incorporating identified individual therapy needs)

Includes

All benefits of A1 excluding one of the Core Therapies.

Excludes

Continuous 1:1 staff supervision over and above that which is naturally provided as part of the core service, any 1:1 staffing by a Support Worker will be charged at a standard hourly rate.

Band A3

Continuing Rehabilitation and Review (incorporating specifically identified therapy disciplines)

Includes

Includes benefits of A1 but excluding two of the Core Therapies.

Excludes

Continuous 1:1 staff supervision over and above that which is naturally provided as part of the core service, any 1:1 staffing by a Support Worker will be charged at a standard hourly rate.

Band A4

Continuing Rehabilitation and Review (incorporating identified therapy disciplines)

Includes benefits of A1 but excluding three of the Core Therapies.

Excludes

Orchard House

Orchard House can provide 22 placements to men with an acquired brain injury.  The focus of admission is for the interdisciplinary team to assess residents' needs around their medical, physical, cognitive, psychological, behavioural and communication skills with the ultimate aim of maximising independence.

Cunningham House can provide 18 placements to men with an acquired brain injury.  Similar to Orchard House, the interdisciplinary team will work together to assess residents' needs with the ultimate aim of maximising independence.

Hilltop House

Hilltop House can provide 9 placements to men with an acquired brain injury.  Throughout assessment the team will develop unique and personalised programmes of rehabilitation for residents and simultaneously provide comprehensive and robust clinical progress updates with the aim to facilitate successful discharge.

The Lodge

The Lodge can provide 4 placements for men who require a service that will meet their ongoing physical and psychological needs.  The service is suitable for those who are able to demonstrate a higher level of risk management who require support to build on independence in all activities of daily living.
